Feedback & Reflection Worksheet

Mentor observation  •  student reflection  •  safe independent travel

Student Name: Date: Mentor / Trainer: Route / Trip:
How to use this worksheet. The mentor fills Part 1 first, right after the trip. Then mentor and student work through Parts 2 and 3 together — always starting with what went well.
PART 1

Mentor Observation Summary

Fill this first — use your Travel Training Log phase notes for accuracy. Be factual, specific, and balanced.

1.Key successes & strengths observed today (be specific — phases, exact behaviors, independence level):
Clarify — Focus on facts and wins to build the student's confidence and self-awareness. Name exact moments ("scanned both directions before crossing," "asked the driver clearly") rather than general praise.
2.Key areas for growth / challenges highlighted (Top 3 — keep factual, observable, non-judgmental). These become the focus of the student's reflection below:
a.
b.
c.
Clarify — Distill from your detailed notes or the Log. Describe what you saw, not a verdict — "phone was out while boarding" rather than "not paying attention."
3.Safety concerns or notes (check one):
☐  None     ☐  Minor (addressed on trip)     ☐  See Log for details
PART 2

Student Reflection & Self-Awareness

Complete this section together — start with the positives, then look at each growth area one at a time.

4.What went well today? What are you proud of doing or accomplishing?
Clarify — There is a lot to be proud of. Think about a moment you handled on your own — a stop you recognized, a question you asked, a time you stayed calm. Say it out loud first, then write it down.

Now reflect on the growth areas one at a time. This helps us understand what happened and plan real improvements.

5.Challenge 1:
What happened? (Describe in your own words)
What were you focusing on or thinking about at that moment?
What can we do differently or better next time? (Your idea + mentor strategy)
Help or reminders I need from my mentor next time:
Clarify — Nobody is in trouble here. Tell the story like a movie: what happened just before, during, and after. The "what were you thinking?" question is not a test — it just helps us plan what to do next time.
6.Challenge 2:
What happened? (Describe in your own words)
What were you focusing on or thinking about at that moment?
What can we do differently or better next time? (Your idea + mentor strategy)
Help or reminders I need from my mentor next time:
Clarify — Share your own idea for next time before your mentor adds theirs. Concrete plans work best — "phone stays in my pocket until my stop" beats "pay more attention."
7.Challenge 3:
What happened? (Describe in your own words)
What were you focusing on or thinking about at that moment?
What can we do differently or better next time? (Your idea + mentor strategy)
Help or reminders I need from my mentor next time:
Clarify — If three challenges feels like a lot, that is okay — leave this one blank. It is better to plan two changes well than to rush through three.
PART 3

Goals, Practice Plan & Progress

Work on this together — make each goal specific, observable, and doable, based on the challenges above.

8.Our top 3 goals for the next trip (specific, observable, with success criteria):
a.
b.
c.
Clarify — A good goal tells you exactly how you will know you did it. "Get off at the right stop" is fuzzy; "watch for the library sign and press the stop button one stop early" is a goal you can actually see happen.
9.Personal goal I want to work on:
10.Mini-lessons or practice activities for next session (role-play, visuals, repetition, video examples, etc.):
Clarify — Pick the one thing you most want to get better at — this goal is yours. Then match a practice activity to it: nervous about asking the driver? Role-play the question a few times before the next trip.
11.Overall independence rating today (circle one):
1  •  2  •  3  •  4  •  5

(5 = Fully ready to travel alone with minimal support)

Clarify — This rating is a snapshot of today, not a grade on you as a person. Talk about it together — comparing today to your own last trip on this route is what matters, not a perfect score.
12.Progress since last session on this route:

How to run this feedback & reflection session with your student

1. What this worksheet builds

It turns one transit trip into safe, independent travel plus real reflection. The student learns to notice their own successes and challenges, think about what they were doing in the moment, and help set concrete goals for the next trip. The aim is student-centered growth and self-awareness — not a score.

2. How to run it

3. Using the Clarify hints

Let the student attempt each section first; read the Clarify box aloud only if they stall. The hints coach strategy ("tell it like a movie," "make the goal something you can see happen"), never the answers. Over time, ask the student to say the idea back before they read it — that is the skill becoming their own.

4. Supporting the student

5. Reviewing & the next trip
